* Screening rate of 80% or higher determined by the Health Resources and Services Administration’s 2015 Uniform Data System (UDS) measure or by community health center self-report.

** Screening rate of 80% or higher determined by the National Committee for Quality Assurance (NCQA) using the colorectal cancer HEDIS measure. The lists of commercial and Medicare Advantage plans is based on plans that had achieved 80% in 2016 (the latest data available).

*** Screening rate of 80% or higher determined by primary care practice or health system self-report.

The goal of the 80% by 2018 Hall of Fame webpage is to acknowledge the work of 80% by 2018 partners that report reaching 80% of adults aged 50 and older screened for colorectal cancer. The NCCRT relies on an honor system for these self-reported achievements. Publicly reported screening rates will be updated as new data is reported. NCCRT will ask organizations that self-report achieving and 80% rate to confirm their place on the list on an annual basis. For questions about the colorectal cancer screening rate of any individual system or practice, please contact the organization directly.

Organizations that are listed but have not yet received a web badge should complete the submission form to receive the badge by email: Submit your 80% Screening Rate Success.
