Guide to High Performing Models
This guide describes four programs that have been identified as High Performing Models by the NCCRT, based on their demonstrated success in delivering colorectal cancer screening and follow up care to uninsured, underinsured and low-income adults in their respective communities.
This guide is organized around the following tasks and potential barriers that new community coalitions may face in developing a program to improve access to colorectal cancer screening:

- Management, Roles and Responsibilities
- Coordination of Patient Care
- Resources and Funding
In each section, readers will find a discussion of how model programs have addressed barriers and achieved success, as well as specific examples of model programs “in action.”
The Appendix at the end of the guide provides examples of forms, logic models, flow charts, and other materials that further illustrate how the model programs have executed the principles described in this guide.
